While the use of sirolimus-eluting stents (SES) is less invasive and safer than bare metal stents (BMS) in the treatment of coronary artery disease (CAD), SES are not as effective as BMS. We compared clinical outcomes among patients with CAD treated with SES and BMS and investigated the potential factors associated with the outcomes of patients with SES. In total, 1,111 patients who underwent PCI with SES (n=844) or BMS (n=267) were identified and analyzed. The patients treated with SES were older, and had more risk factors than those treated with BMS. The incidence of target lesion revascularization (TLR), target vessel revascularization (TVR) and stent thrombosis (ST) was significantly higher in SES group compared to BMS group. Multivariate analysis showed that in-hospital TLR was significantly associated with in-hospital TVR and myocardial infarction (MI) in SES group. In BMS group, in-hospital TLR and TVR were significantly associated with in-hospital MI, and ST was significantly associated with in-hospital MI and in-hospital death. The independent predictors for in-hospital TLR in SES group were in-hospital MI, history of PCI, and history of MI. In BMS group, in-hospital TLR was independently associated with in-hospital MI, while in-hospital TVR was independently associated with in-hospital MI, history of PCI, history of MI and stent size. The independent predictors for in-hospital TLR and TVR in BMS group were in-hospital MI and stent size. Stent size significantly influenced in-hospital TLR in SES group, whereas stent size was the only factor influencing in-hospital TVR in BMS group. In conclusion, larger stent size was associated with lower rate of ST and in-hospital death and therefore may benefit SES patients in whom complications increase after PCI, including in-hospital MI, death and TLR.
